Vertical Lip Lines…smokers/pout lines

Oral Commissure Lines….mouth frown 

The area at the corner of the mouth where the upper lip meets the lower lip is called the oral commissure.  Oral commissures usually experience a gradual depression over time.  In many individuals, the depression contributes to the formation of wrinkles and lines, as well as contributing to a more angry or stern appearance.  Oral commissure lines may extend downward from the corners of the mouth,  giving an individual an unnatural puppet or marionette-type appearance.  In other instances, oral commissure lines may fan out from the corner of the mouth. 

 

Oral commissure lines are almost always treated with advanced dermal fillers.  In instances where individuals have not developed deeper lines or folds,. These fillers help push up and buttress the folds.  Dermal fillers placed in other areas of the face (the upper cheeks for instance) may also give a subtle lift to the oral commissure area, which will help alleviate some of the oral commissure lines

 

Accordion Lines….smile lines extending to the lips to cheeks

They tend to be prominent in people with large mouths and in those people who are on the thinner side.  Accordion lines appear because of the muscle action of the large orbicularis oris muscle which surrounds the mouth.

 

Accordion lines are treated well by volumising with dermal fillers.

 

The next step after dermal fillers would be to consider a facelift or S-lift procedure with a plastic surgeon.  At Palm Clinic we prefer the more natural appearance of volumising with dermal fillers.

Marionette lines…………puppet lines

Marionette lines appear commonly as we age. These are the lines that run downwards from the corner of the mouth. They can make you appear to be frowning and can make you appear sad, tired or unhappy even when you are not! As we age, these lines can become deeper and sagging skin from loss of mid-face cheek fat can produce an overhang of sagging cheek skin that may make them appear even deeper. To help to restore some of the lost volume and elevate the depth of the furrows so that the darker shadows that accentuate the depth of these folds and furrows are not as evident. The result is a youthful, more rested look that people notice, but typically, they don’t know something was done. It also helps to avoid the drawn or tight look sometimes seen after some facelifts and gives a soft natural look.

 

 Accordian Lines …….smile lines 

Believe it or not  there is a name for those cheeky little lines that can persist on your lower cheek area. 

 

Accordion lines are the static wrinkles that are present even when you’re not smiling on the outer sides of your mouth. 

Facial changes occur over time and these ‘not quite charming’ dimples in the lower face are lines that are the result of repeated muscle movements that form facial expressions (especially smiling).


In addition, a loss of volume in the same area caused by a decrease in naturally occurring sugars and fat under the skin contributes to their visibility.


Gravity, sun damage and other environmental and genetic factors can also play a part. 

 

Restoring the naturally occurring sugars that help create volume under the skin can diminish the appearance of these pesky accordion lines.  Dermal  Fillers are the solution.


Over time these fillers have become more sophisticated, they lend themselves to very superficial work through to deeper volumisation, as they are injected artistically to ensure the most natural appearance possible.


Sagging jawline (jowls) …. notching along the jaw line

 

Some people get two little indentations that occur on the jaw line directly below the outside corners of their mouth. These indentations, or notches, interrupt the smoothness of the jaw line and can give it a shape that looks a little bit like an "S."  Medically this is called prejowl sulcus.  ideally we want our jaw line to be a smooth line from the ear to the chin.  Different things can cause these notches, and the treatment will differ depending on your particular situation.

If the cause of this notching is thinning of the fat pad in the chin area or over-activity of the muscle that holds down the corners of the mouth, your best option is filling the area with Juvederm or Restylane. A drop of Botox in this area can also help. 

 

Chin Strengthening/Projection …..effective alternative to implants

 

A number of patients desire improvement in the appearance of a poorly defined, underprojected chin. A strong chin can help add definition and balance to the other facial features. For patients who desire a non-surgical solution or are interested in seeing potential results before undergoing surgery, chin augmentation with filler is an excellent option.  The chin is one of defining features of the face, giving shape to the face, and neck as well. And with ageing, the chin tends to lose volume and projection. Fortunately, a natural chin enhancement can help to return the chin to a more youthful state without looking ‘fake’ or unnatural.


Even with a modest amount of filler who simply desire some forward projection of their chin. can have a noticeable improvement. This can also help to define the jawline, and in many cases, provide the patient with a more tightened look to their neck. But even for patients having a small amount of dermal filler can provide impressive improvement.
